Major Hospital Foundation offers friends and family a way to memorialize their loved ones through the Tree of Life. The Tree of life, a beautiful and dignified tribute, is located in the lobby of Major Hospital. Its design incorporates the names of people who have been remembered though a gift to the Major Hospital Foundation.







Gift Level
Gift Level Description
SUPPORTER:
$250 to $1,000.		 A yellow or gold brass leaf will be engraved and added to the Tree of Life.
ADVOCATE:
$1,001 to $10,000		 An antique bronze leaf will be engraved and added to the Tree of Life.
PATRON:
$10,001 to $20,000		 An acorn will be engraved and added to the Tree of Life.
BENEFACTOR: $20,001 to $50,000 A small stone will be engraved and added to the Tree of Life.
CAPITAL CLUB: $50,001 to $100,000 A medium stone will be engraved and added to the Tree of Life.
PRESIDENT’S CLUB: $100,001 to $200,000 A large stone will be engraved and added to the Tree of Life.
FOUNDER’S CLUB: $200,001 and over An oak tree will be engraved and added to the Tree of Life.
